    To submit a course request using this form:
(1) Follow the instructions to enter the requested information
(2) When form is complete click "Submit" at the bottom of the form.

     
  • Note: For requests for Special Topics courses:
            (a) A course description needs to be included in the text field below.
    and then
            (b) A course outline/syllabus which includes course content, prerequisits,
                text, etc., must be submitted under separate cover with a Special Topic request form before course approval can
                be requested. [Both the U of M Request for Special Topics form, and the Standard Form for Course Syllabi to accompany it, can be filled in online and printed from the site. Go to the Special Topics Request from this link to open the form in this frame, or from the main page or from the link on the course list at the left to open the form in a new browser window.

  • To use this form to request a readings or other special section, simply enter the course number in the first course block and use the special topics field to describe the request.
  • To use this form to request a section of an existing catalogue course not presently on the proposed schedule, simply enter the course number a course block, a proposed time and use the special topics field to describe the request. Include requests for Independent Study, Dissertation, Thesis, etc., for Grad advisees here and indicate the number of hours.
