The Patriot Medal is awarded only by a state society. It is the highest award given for service at the state or chapter level. This medal is presented only to individuals and may be received ONLY once. It represents long, faithful and outstanding service at the state and/or chapter level. Service at the national level can not be considered in making the award.

State Medal Of Distinguished Service Medal
Form Description:
The State Medal of Distinguished Service, authorized in 1998, is the highest Medal that can be awarded by a State Society President during each year of his term of office. It is presented to a Compatriot for distinguished service rendered to the State Society on the state level during a State Society’s President’s term of office. The Medal should be presented at the State Society annual meeting at the conclusion of the President’s term of office. The State Medal of Distinguished Service is presented by the incumbent State Society President and does not require other approval.

The highest award presented to a member by the National Society. It is presented for distinguished service rendered to the SAR on the National level. Members are selected by the Minuteman Committee from submitted applications. The Committee is appointed by the President General from previous recipients of the award. It is presented during the Annual Congress to no more than six recipients per year. More information can be found in the SAR Handbook.

The Minuteman Nomination Form linked below is a form fillable PDF. It will let you fill-in the form on-screen, then print-out both form and data at the same time on 8.5 by 14 inch paper. You may save the data along with the program by using the "Save As" option and using a unique name to save it.
