This form is used to request that SAR headquarters staff do ONE of the following:

• a) search for and send you the latest application filed on a specific patriot ancestor.
• b) send you a "record" copy of the application for a specific member by name or by national number. (A "record copy" is a copy of the approved application, which has annotations that were made during the approval process.)

Please be aware that we do not have backup documentation for those applications filed before 1978. The approximate Member National Number cutoff for the documentation files is numbers prior to #11400.
