This form is used to request that SAR headquarters staff do ONE of the following: a) search for and send you the latest application filed on a specific patriot ancestor. b) send you a "record" copy of the application for a specific member by name or by national number. (A "record copy" is a copy of the approved application, which has annotations that were made during the approval process.)
