This file allows the user to produce an application form that is (within the limits of the printer being used) identical to the current pre-printed NSSAR Membership Application Form, Form #0915.  There is a similar form for preparation of Youth and Youth Life Membership Applications.

27 Feb 2007 - An updated and far more powerful interactive version of the 04 Mar 2003 Universal form, sa03915u.pdf, that replaced the above forms, is now available for use by registered users at the author's web site.   Although the new interactive form, sa03915u_itext.pdf, when printed appears the same as the current pre-printed Universal form, it has been assigned a new issuance date, 27 Feb 2007, to distinguish it from application forms generated by using other software processes and the previous PDF forms from the author's website.

This method employs Adobe Portable Document Format (TM) and the (free) Adobe Acrobat Reader (TM).  Thus, it works equally well on Windows, MAC, and Linux operating systems.  No other software programs for SAR applications enjoy this portability and easy access across multiple operating systems.
