SAR History Education Patriotism Genealogy
The Following page lists the various Forms and Publications that the NSSAR uses. By using the drop down list to the left, you can select the type of form that you are looking for.
If you are looking to report a Compatriots death, you can use the Compatriot Death Report Form.
If you are looking to report a change in State of Chapter level officers, you can use the Officer change report Form.

| Minuteman Nomination |
The highest award presented to a member by the National Society. It is presented for distinguished service rendered to the SAR on the National level. Members are selected by the Minuteman Committee from submitted applications. The Committee is appointed by the President General from previous recipients of the award. It is presented during the Annual Congress to no more than six recipients per year. More information can be found in the SAR Handbook. The Minuteman Nomination Form linked below is a form fillable PDF. It will let you fill-in the form on-screen, then print-out both form and data at the same time on 8.5 by 14 inch paper. You may save the data along with the program by using the "Save As" option and using a unique name to save it. |

| National Reconciliation Template |
This form is to be used by States. In addition to providing a standardized format for reporting all the changes in membership year over year, the new Reconciliation Form also calculates the amount of dues that the society owes to the National Office. In future years, the form can be updated to include any changes in dues or membership categories. There are instructions and a Chapter format as well. (Both Can be found in the forms section)
he 2013 template has been tested by a number of state society secretaries across the nation to find any formula errors and to identify potential issues. The template that is now available is the result of this testing. The version of the template that you should be using has the following time / date stamp on the Cover Sheet: “Last Updated: 20 May 2013 – 800 hours MCA”.
The major change for this year is that all positive additions to the society are entered on a single page. Also, Memorial memberships can now be correctly entered as both an addition and subtraction.
Please note that if you have been keeping track of the 2013 membership on the previous form that data does not need to be entered over again. If desired, that data should be COPIED & PASTED into the new template. DO NOT CUT & PASTE as that will corrupt the embedded formulas in the spread sheet.

| NSSAR Genealogical Policies |
The Genealogy Committee of the NSSAR is responsible for forming the genealogy policies of the organization. Policies are formed by official action of the Committee during its meetings. A policy is becomes effective upon adoption and remains in effect until the policy has been completed or rescinded. The Genealogy Committee works closely with the Genealogist General and the NSSAR Genealogy Staff in the administration of the policies. This on-line document is updated each time policy changes are adopted by the Committee so users are encouraged to download updated copies frequently. |

This is a large version of the Library Handout that is intended to be used to hand out or place on a counter or at a booth. Members to have the ability to print off their own copies for events etc. This is cost effective and also a quicker turn around time for members that need this last minute. Life Membership may now be purchased at a rate that depends on your age. In September 2009 the fee scale was approved and it appears on the Life Membership Application form. Note: This fee covers the National dues only, so you will still have to pay the State Society's annual fee to be an active member of the SAR. |

| Oration Endowment Contribution Form |
The Rumbaugh Orations Contest’s National level awards are not currently competitive among nationwide contests. Currently, the program disperses $11,000 annually where competitive levels are closer to $30,000. The Orations Committee is seeking to rectify this disparity with an enhanced endowment. Joseph Warren, a Boston doctor, was the orator selected to commemorate the anniversary of the Boston Massacre. Because the orator would be heckled by unforgiving British officers and sympathizers, Warren bravely presented his speech with a steady and firm voice. Warren’s stirring oratory skill recruited more patriots and moved others to support the Revolution. |

The Patriot Medal is awarded only by a state society. It is the highest award given for service at the state or chapter level. This medal is presented only to individuals and may be received ONLY once. It represents long, faithful and outstanding service at the state and/or chapter level. Service at the national level can not be considered in making the award. |

Updated 01 April 2013 This is the combined form for both the President General's Cup and the Americanism Award. The Form includes the score sheet as well as directions about the contest. Any questions should be directed to the Americanism Committee. This is a new version of the form that was re-worked by the americanism committee. The purpose of this pamphlet, provided by the Sons of the American Revolution Reenactors Committee (SARRC), is to provide suggestions, recommendations, and some basic guidelines for co-sponsored events and activities between the Sons of the American Revolution and the American Revolutionary War historical reenactment community. | Replacement-Duplicate Membership Form |
Use this form to request a replacement or duplicate membership certificate. Download the form fill it out, and then mail it back to the NSSAR headquarters.
Of note, By default, all certificates contain electronic signatures of the current President General and Secretary General, as well as the member’s State Society at the time of approval. To obtain State officers’ signatures, membership certificates will first be mailed to the State Society where the membership originated (even if the member later transferred to a different State Society). The certificate will then be forwarded to you by that State Society. Supplemental certificates will be mailed directly to you.

| Research request Form |
The Research Service is an hourly-based search of the various sources available within the NSSAR Genealogical Research Library. The various sources available may include but are not limited to NSSAR applications, documentation records, family Bible, Ancestry, and other Genealogical Societies. Each separate hourly request will receive 10 standard pages of photocopied material (any additional pages will be billed to the requestor at $0.50/page plus mailing cost). Any unused standard pages of the initial 10 do not carry over to a new research request unless the new research request is a continuation of the original research request. The minimum request is for one hour. You will receive a status report after the first hour of research is completed with follow up reports after every 3rd hour or the end of the research project. The Research Service does NOT accept requests via email or over the telephone. All requests must include a signed and dated Research Letter of Agreement, completed Research Request Form and payment, along with additional material enumerated in the Research Letter of Agreement at the time of requests. |
