The Following page lists the various Forms and Publications that the NSSAR uses. By using the drop down list to the left, you can select the type of form that you are looking for.

If you are looking to report a Compatriots death, you can use the Compatriot Death Report Form.

If you are looking to report a change in State of Chapter level officers, you can use the Officer change report Form.

NSSAR Genealogical Policies

 

The Genealogy Committee of the NSSAR is responsible for forming the genealogy policies of the organization. Policies are formed by official action of the Committee during its meetings. A policy is becomes effective upon adoption and remains in effect until the policy has been completed or rescinded.
 
The Genealogy Committee works closely with the Genealogist General and the NSSAR Genealogy Staff in the administration of the policies.
 
This on-line document is updated each time policy changes are adopted by the Committee so users are encouraged to download updated copies frequently.

NSSAR Form 0921: Member Transmittal Form

(Form 0921, last revised Oct 2012) State Secretaries and others use this form to accompany materials dealing with member status. This file has fully active form fields. You will need Adobe Acrobat 6.0 or higher.  Note: Once filled in please print this form on canary yellow paper so it is less likely to get lost in the documentation that accompanies the application.

NSSAR Form 0919: Transfer of Membership

Use this if you wish to change your primary membership affiliation to a new state. Note: If you have paid National dues to the State which you are leaving, you do not need to pay National dues for that same year to the State to which you are transferring.

NSSAR Form 0912: Requirements for Preparation of Applications

 

Form 0912 is being updated, but older versions are still available from Merchandise to assist applicants. Please see the on-line NSSAR Genealogy Policy Manual and the Registrar’s Instruction Manual for more complete information and guidance on preparing applications.

National Reconciliation Template

This form is to be used by States. In addition to providing a standardized format for reporting all the changes in membership year over year, the new Reconciliation Form also calculates the amount of dues that the society owes to the National Office. In future years, the form can be updated to include any changes in dues or membership categories. There are instructions and a Chapter format as well. (Both Can be found in the forms section)

 

he 2013 template has been tested by a number of state society secretaries across the nation to find any formula errors and to identify potential issues.  The template that is now available is the result of this testing.  The version of the template that you should be using has the following time / date stamp on the Cover Sheet:  “Last Updated: 20 May 2013 – 800 hours MCA”.

 

The major change for this year is that all positive additions to the society are entered on a single page.  Also, Memorial memberships can now be correctly entered as both an addition and subtraction. 

 

Please note that if you have been keeping track of the 2013 membership on the previous form that data does not need to be entered over again.  If desired, that data should be COPIED & PASTED into the new template.  DO NOT CUT & PASTE as that will corrupt the embedded formulas in the spread sheet. 

 

As always, if there are any issues, please contact the National Office and they will forward all questions and issues to the appropriate volunteers for follow up.

Minuteman Nomination

The highest award presented to a member by the National Society. It is presented for distinguished service rendered to the SAR on the National level. Members are selected by the Minuteman Committee from submitted applications. The Committee is appointed by the President General from previous recipients of the award. It is presented during the Annual Congress to no more than six recipients per year. More information can be found in the SAR Handbook. The Minuteman Nomination Form linked below is a form fillable PDF. It will let you fill-in the form on-screen, then print-out both form and data at the same time on 8.5 by 14 inch paper. You may save the data along with the program by using the "Save As" option and using a unique name to save it.

How We Became the United States of America: The Values that Built Our Nation Teacher Registration Form

 

Objective:
    The three year professional development program will enrich and strengthen fifth grade social studies teachers’ understanding of the values and history of the American Revolution.  Along with content programs educators will be supplied with interactive methods of teaching to incorporate materials about the period into their classrooms, beyond basic lecturing.

 

Content is intended to develop the following knowledge and skills:
  • An understanding of life during that period in our history – both for common citizens and for those who actually fought in battles.
  • An understanding of the documents and the symbols, Constitution, American Flag, Liberty Bell, that were developed during that period.
  • The ability to understand the difference between various Revolutionary War time periods.
  • The ability to understand how people have addressed broad problems.
  • An understanding of the chronology of events during the late 1700s and early 1800s.
 

 

Program benefits to Educators:
  • Esteemed professors of History presenting small group sessions focused on the founding of our nation.
  • Free housing for 2 nights at beautiful Laws Lodge – Conference Center at Louisville Seminary
  • Transportation stipend for teachers driving to Louisville.
  • Entrance Fee to Riverside Farnsley-Moremen Landing.
  • All meals and snacks for two day seminar.
  • Free classroom materials for educators to enrich their school’s history curriculum.

  • Teachers must be of full-time teaching status

Friends of the Library Application

The goal of the Friends of the SAR Library is to raise $150,000 for the Library Endowment Fund. Also, the Society Chairman of the Friends of the SAR Library will coordinate the donation of books, microfilms, etc. direct to the NSSAR Library and will promote the NSSAR Library within his Society so that all SAR Members are familiar with it and its programs.
