NSSAR Forms and Publications

The Following page lists the various Forms and Publications that the NSSAR uses. By using the drop down list to the left, you can select the type of form that you are looking for.

If you are looking to report a Compatriots death, you can use the Compatriot Death Report Form.

If you are looking to report a change in State of Chapter level officers, you can use the Officer change report Form.

SAR Application (PDF Version)	SAR-APP-2012-05-08.pdf	This file allows the user to produce an application form that is (within the limits of the printer being used) very similar to the current pre-printed NSSAR Membership Application Form, Form #0915. Before Printing, you may need to change the Page Setup to Legal Size paper. When printing, Print page 1, then reinsert the paper into your printer and print Page 2 on the back side. The form and data can be saved to your hard drive for editing later or to use when filing a Supplemental Application which uses much of the same lineage. State Medal Of Distinguished Service Medal	State-Distinguished-Service-2009-11-25.pdf	The State Medal of Distinguished Service, authorized in 1998, is the highest Medal that can be awarded by a State Society President during each year of his term of office. It is presented to a Compatriot for distinguished service rendered to the State Society on the state level during a State Society’s President’s term of office. The Medal should be presented at the State Society annual meeting at the conclusion of the President’s term of office.
