NSSAR Forms and Publications

The Following page lists the various Forms and Publications that the NSSAR uses. By using the drop down list to the left, you can select the type of form that you are looking for.

If you are looking to report a Compatriots death, you can use the Compatriot Death Report Form.

If you are looking to report a change in State of Chapter level officers, you can use the Officer change report Form.

National Reconciliation Template

This form is to be used by States. In addition to providing a standardized format for reporting all the changes in membership year over year, the new Reconciliation Form also calculates the amount of dues that the society owes to the National Office. In future years, the form can be updated to include any changes in dues or membership categories. There are instructions and a Chapter format as well. (Both Can be found in the forms section)

For those who have already entered data on the previous version, there is no need to reenter in the new version as the National Office will accept either version this year (2012-2013).

 

Please be advised that on the State Society Annual Reconciliation Report form that you should NOT place a space at the end of ANY data entry.  Placing a space will cause the form to reject the data entry and thus not calculate totals correctly. 
 
Again, the directions are specific about what can be entered – if it says “Y” or “N”, then type “Y” or “N” and not “Y(space” or “N(space)”.

Oration Endowment Contribution Form

 

 

The Rumbaugh Orations Contest’s National level awards are not currently competitive among nationwide contests. Currently, the program disperses $11,000 annually where competitive levels are closer to $30,000.
 
The Orations Committee is seeking to rectify this disparity with an enhanced endowment.
 
Joseph Warren, a Boston doctor, was the orator selected to commemorate the anniversary of the Boston Massacre. Because the orator would be heckled by unforgiving British officers and sympathizers, Warren bravely presented his speech with a steady and firm voice. Warren’s stirring oratory skill recruited more patriots and moved others to support the Revolution.

NSSAR Life Membership Application

Life Membership may now be purchased at a rate that depends on your age. In September 2009 the fee scale was approved and it appears on the Life Membership Application form. Note: This fee covers the National dues only, so you will still have to pay the State Society's annual fee to be an active member of the SAR.

SAR Application (PDF Version)

This file allows the user to produce an application form that is (within the limits of the printer being used) very similar to the current pre-printed NSSAR Membership Application Form, Form #0915. Before Printing, you may need to change the Page Setup to Legal Size paper. When printing, Print page 1, then reinsert the paper into your printer and print Page 2 on the back side. The form and data can be saved to your hard drive for editing later or to use when filing a Supplemental Application which uses much of the same lineage. Remember to use the "Save As" Command, and change the name of the file.

(Form 0921, last revised Oct 2012) State Secretaries and others use this form to accompany materials dealing with member status. This file has fully active form fields. You will need Adobe Acrobat 6.0 or higher.  Note: Once filled in please print this form on canary yellow paper so it is less likely to get lost in the documentation that accompanies the application.

Research request Form

 

 

The Research Service is an hourly-based search of the various sources available within the NSSAR Genealogical Research Library. The various sources available may include but are not limited to NSSAR applications, documentation records, family Bible, Ancestry, and other Genealogical Societies.
 
Each separate hourly request will receive 10 standard pages of photocopied material (any additional pages will be billed to the requestor at $0.50/page plus mailing cost). Any unused standard pages of the initial 10 do not carry over to a new research request unless the new research request is a continuation of the original research request.
The minimum request is for one hour. You will receive a status report after the first hour of research is completed with follow up reports after every 3rd hour or the end of the research project.
 
The Research Service does NOT accept requests via email or over the telephone. All requests must include a signed and dated Research Letter of Agreement, completed Research Request Form and payment, along with additional material enumerated in the Research Letter of Agreement at the time of requests.

Replacement-Duplicate Membership Form

 

Use this form to request a replacement or duplicate membership certificate.  Download the form fill it out, and then mail it back to the NSSAR headquarters.

 

Of note, By default, all certificates contain electronic signatures of the current President General and Secretary General, as well as the member’s State Society at the time of approval. To obtain State officers’ signatures, membership certificates will first be mailed to the State Society where the membership originated (even if the member later transferred to a different State Society). The certificate will then be forwarded to you by that State Society.

Supplemental certificates will be mailed directly to you.

Annual Reconciliation Instructions

The Task Force for Registration, Education & Reporting has created a single, easy to use Reconciliation Form Template for calculating and reporting both active membership and accurate dues payment amounts for State Societies. This standardized Reconciliation Form is intended to speed up the data gathering and reconciliation process and, at the same time, reduce the man hours currently required in reconciling the membership of the National Society by the National Office. It will also expedite each state society’s payment of dues to the National Office.

Color_Guard_Reporting

 

SAR Color Guardsmen who believe they qualify for the Silver Color Guard Medal must complete the appropriate form, setting forth their qualifications.  This form is available from the State, District or National Color Guard Commander, and must be submitted to the District and/or National Color Guard Commander for authorization to award the Silver Color Guard Medal.

How We Became the United States of America: The Values that Built Our Nation Teacher Registration Form

 

Objective:
    The three year professional development program will enrich and strengthen fifth grade social studies teachers’ understanding of the values and history of the American Revolution.  Along with content programs educators will be supplied with interactive methods of teaching to incorporate materials about the period into their classrooms, beyond basic lecturing.

 

Content is intended to develop the following knowledge and skills:
  • An understanding of life during that period in our history – both for common citizens and for those who actually fought in battles.
  • An understanding of the documents and the symbols, Constitution, American Flag, Liberty Bell, that were developed during that period.
  • The ability to understand the difference between various Revolutionary War time periods.
  • The ability to understand how people have addressed broad problems.
  • An understanding of the chronology of events during the late 1700s and early 1800s.
 

 

Program benefits to Educators:
  • Esteemed professors of History presenting small group sessions focused on the founding of our nation.
  • Free housing for 2 nights at beautiful Laws Lodge – Conference Center at Louisville Seminary
  • Transportation stipend for teachers driving to Louisville.
  • Entrance Fee to Riverside Farnsley-Moremen Landing.
  • All meals and snacks for two day seminar.

  • Teachers must be of full-time teaching status

The National Society of the Sons of the American Revolution (known hereafter as NSSAR) is a not for profit, tax-exempt organization. Maintenance of its tax-exempt status is important for both continued financial stability, public and member support. Therefore, the Internal Revenue Service, as well as other regulatory agencies, tax officials and other stakeholders view the policy and operations of the SAR as a public trust, which is subject to scrutiny by and accountable to such authorities as well as its constituents.

PG-Cup-Americanism-Scoresheet

This is the combined form for both the President General's Cup and the Americanism Award. The Form includes the score sheet as well as directions about the contest. Any questions should be directed to the Americanism Committee.  This is a new version of the form that was re-worked by the americanism committee.

Friends of the Library Application

The goal of the Friends of the SAR Library is to raise $150,000 for the Library Endowment Fund. Also, the Society Chairman of the Friends of the SAR Library will coordinate the donation of books, microfilms, etc. direct to the NSSAR Library and will promote the NSSAR Library within his Society so that all SAR Members are familiar with it and its programs.
